Appointments are scheduled within one business day
of referral. Evening appointments are available. Initial evaluations
with treatment
plans and progress reports are communicated to referring practitioners
on a regular basis.
We are providers for Worker's Compensation, most managed care
plans, private insurance plans, and Medicare. Our office staff
will assist a patient in determining their insurance benefits
for rehabilitation
services.
Treatment is offered at two locations: at Homestead Physical
Therapy, 4922 Columbia Road in Cedarburg, and at NorthShore Physical
Therapy,
