Donald Olsen, EdD. PT. OCS
Northshore/ Homestead Physical Therapy
Donald is the Private Practice owner of both Northshore Physical Therapy and Homestead Physical Therapy. He received a B.S. degree from Eastern Michigan University; a graduate certificate in Physical Therapy from Columbia University, Masters of Physical Therapy from Emory University, and a Doctorate degree from the United States Sports Academy. Don has over 27 years of experience in treating orthopedic, TMJ, sports, and industrial cases. He has taught at Marquette University and is a member of the American Physical Therapy Association, Orthopedic and sports Medicine Sections, the Wisconsin P.T. Assoc., World Confederation of P.T., and the American College of Sports Medicine. He has attended numerous postgraduate courses in orthopedic and manual physical therapy.

Krista Formanek, MS, PT, DPT

Northshore Physical Therapy

Krista received her Bachelor of Science degree in biology and her Masters of Science in Human Performance from the University of Wisconsin La Crosse. She then completed her Doctorate in Physical Therapy from the University of Wisconsin La Crosse.  She has clinical experience in a variety of outpatient settings including private practice, clinics, and hospitals.  Krista also has experience as a personal trainer and yoga and pilates instructor.

 
Joseph Tepp, DPT
Northshore/ Homestead Physical Therapy
Joseph received his Bachelor's Degree in Biology-pre Physical Therapy from Carroll University, WI and his Doctor of Physical Therapy from Southwest Baptist University, MO. Currently an orthopedic PT resident with Evidence in Motion. He enjoys using a manual/manipulative hands-on approach, treating the whole person to reduce pain and promote wellness. Dr. Tepp enjoys treating all areas with a special interest in the spine. He is a member of the American Physical Therapy Association, Wisconsin Physical Therapy Association, and American Academy of Orthopedic Manual Physical Therapists.
Ann John, OTR
Northshore /Homestead Physical Therapy
Ann received an Occupational Therapy Degree from UW-Milwaukee in 1987. She is registered and certified by the State of Wisconsin and is a member of the Wisconsin and American Occupational Therapy Associations and Work Section of the AOTA. Ann has over 16 years of experience in occupational therapy. She provides functional capacity testing, job site assessments, and work reconditioning evaluations to maximize functional potential on the job.
Robin Brackett, PTA
Northshore Physical Therapy
Robin received her Associate Degree as a Physical Therapy Assistant from Gateway Technical College. She also has a degree in psychology from Valparaiso University.  She has spent her career specializing in orthopedic injury rehabilitation.
